To receive your manufacturers’ cash rebate, send the completed form and original UPC Bar Code along with a copy of your receipt. Allow up to 45 days to receive the rebate check. Rebates do not include tax. Products must be purchased between January 10, 2011 and January 16, 2011 and submitted by January 31, 2011.
